Can IHS Pay for My Addiction Treatment?

If you are wondering whether IHS will pay for treatment, the answer is often yes. The Indian Health Service may pay for alcohol and drug treatment at an outside treatment center through its Purchased/Referred Care program, as long as your PRC office authorizes the care before you go. This guide explains the rules in plain language, lays out the steps in order, and tells you what to do if you are turned down.

PRC pays for care that IHS cannot provide itself, at a provider IHS has approved, when your PRC office says yes ahead of time. It is not insurance. Your PRC office decides each request based on eligibility, medical priority, and the funds it has that year, and any other coverage you have (Medicaid, Medicare, private insurance) gets used first.

Thundermaker Wellness Center is a confirmed IHS PRC provider for the Nashville Area, so your PRC office can authorize treatment here. The quickest route is to contact your service unit and request treatment at Thundermaker.

What Is Purchased/Referred Care?

Every IHS Area is divided into service units, and each service unit has a Purchased/Referred Care office. That office pays outside providers, such as hospitals, specialists, and treatment centers, for care the IHS clinic cannot provide on its own. Addiction treatment services are a common reason a service unit turns to PRC, because most IHS clinics do not offer them.

PRC funds are limited and set each year, so offices rank requests by medical priority. IHS publishes its IHS medical priority levels (opens in new tab), and the list of covered services is on that page.

Am I Eligible?

IHS sets the eligibility requirements for PRC. You need to meet all of them.

  1. Indian or Alaska Native descentYou are a member of a federally recognized tribe or can show Indian or Alaska Native descent, such as tribal membership or enrollment.
  2. Residency in the PRC delivery areaYou live inside your service unit's PRC delivery area, which covers the reservation, trust land, and bordering counties. Residency is documented on IHS form IHS-976.
  3. NotificationFor non-emergency care, your PRC office must be notified before care begins. For an emergency, notify the office within 72 hours after treatment starts, or within 30 days if you are 65 or older or have a disability.
  4. AuthorizationCare must be authorized by the PRC authorizing official, based on a provider referral, and a purchase order must be issued before PRC pays.
  5. Medical priorityThe service must be medically necessary and fall within the Area's medical priorities. See the IHS medical priority levels (opens in new tab).
  6. Alternate resourcesYou must apply for and use all alternate resources available to you, such as Medicare and state Medicaid. IHS is the payer of last resort.

If you are not sure about any of these, call the PRC office anyway. They will tell you what you are missing. The full requirements are on the IHS PRC eligibility page (opens in new tab).

How Do I Get PRC to Pay for Treatment?

These are the steps in the order that works best. Skipping ahead is where most requests run into trouble.

  1. Contact your service unit firstCall the PRC office at the service unit that serves your tribe and ask for the PRC coordinator or technician. Tell them you are seeking substance use treatment and that you are requesting Thundermaker Wellness Center in Prescott, Arizona, a confirmed IHS PRC provider for the Nashville Area. Ask whether you need a referral from an IHS provider first.Mashpee, for example, only accepts referrals started by its own providers.
  2. Then call ThundermakerOne conversation at (928) 531-1930 confirms the program fits you. Admissions works with your PRC office on the authorization so you are not the one chasing paperwork.
  3. Give the office your documentsProof of tribal enrollment or descendancy, proof of your address, and every insurance or Medicaid card you have. The full list is below.
  4. Apply for other coverage if they askIf you may qualify for Medicaid or another program, the office will ask you to apply. That is normal and does not mean PRC is turning you down.
  5. Wait for the authorizationThe office reviews eligibility, priority, and funds, then issues an authorization or purchase order to the treatment center.
  6. Admit, and keep your PRC office in the loopThe treatment center bills PRC directly. If your stay needs to be extended, the center requests it from the PRC office before the authorized days run out.

Care you receive without notifying your PRC office is usually not paid, even if you would have qualified. Please do not go first and ask later.

What Do I Need to Bring or Send to My PRC Office?

  • Tribal enrollment card or Certificate of Degree of Indian Blood (CIB)
  • Proof of descendancy if you are not enrolled (a parent's or grandparent's enrollment plus birth certificates)
  • Photo ID
  • Proof of current address inside the delivery area (lease, utility bill, mail)
  • Medicaid, Medicare, VA, or private insurance cards, or a denial letter if you applied and were turned down
  • Name, phone, and address of the treatment center and the name of your admissions contact
  • Any referral or assessment from an IHS provider
  • Court paperwork, if a court or probation officer is involved

What If It Is an Emergency?

If you or a family member is in immediate danger, get care first. Then make sure the PRC office is notified within 72 hours of the start of care, or within 30 days if the patient is 65 or older or disabled. Late notification is one of the most common reasons emergency care goes unpaid.

What If I Am Denied?

PRC offices are required to give you a written notice that says why the request was denied and how to appeal. Common reasons are missing documents, living outside the delivery area, not applying for other coverage, or a low priority ranking that year. Most of these can be fixed and resubmitted. Read the deadline on the letter and ask the treatment center's admissions team to help you respond.

What PRC will not do

  • Pay for care it was not told about in advance, outside the emergency rules
  • Pay before your other coverage has been used
  • Guarantee approval because a provider is an approved vendor

What PRC can do

  • Pay the full cost of authorized addiction treatment services when no other coverage applies
  • Extend an authorization when the treatment center requests it in time

Questions People Ask

Does IHS pay for rehab?

IHS may pay for alcohol and drug treatment at an approved outside provider through Purchased/Referred Care when the service unit's PRC office authorizes it in advance and the person meets the eligibility, residency, notification, priority, and alternate-resource requirements. It is not automatic.

Does PRC cover addiction treatment services?

Yes, when authorized. Addiction treatment services are a common PRC referral because most IHS clinics do not offer them. The office decides how many days to authorize, and extensions are requested by the treatment center.

Do I need Medicaid before PRC will pay?

If you may qualify for Medicaid, Medicare, VA benefits, or other coverage, the PRC office will require you to apply. PRC is the payer of last resort and covers what those programs do not.

Can PRC pay for treatment in another state?

Yes. PRC pays approved vendors regardless of where they are located, as long as the vendor is approved for your Area and the care is authorized. Thundermaker in Arizona is a confirmed IHS PRC provider for the Nashville Area, which covers tribes in New York, Massachusetts, and Virginia.

How long does PRC approval take?

It depends on the office and how complete your request is. Requests with documents, a provider referral, and a named vendor with an admission date move fastest. Ask the office for its typical timeline when you call.

What if my tribe's PRC office has never heard of the treatment center?

Ask the treatment center for its vendor information and give it to the PRC office. If the center is not yet an approved vendor for your Area, the PRC office can tell you how to request that it be added.
